We help to identify activities and outcomes that meet your needs. We don’t try to squeeze you into a box that someone else has designed.

Individuality is vital – we know and respect that. Every plan is different. We call this ‘Person First Planning’, because we know that you come first. It is at the core of everything we do. We know that if it is to be successful, it must be your plan, not ours. We don’t tell you what to do, when or how. We help you decide your own program to meet your goals.

You decide what kind of life you would like to have and how we can support you to reach your goals. Your mentor works with you to develop your personal plan and celebrate your successful achievements along the way.

How we help

As approved provider to the National Disability Insurance Agency (NDIA), Chorus can help you test your eligibility for the National Disability Insurance Scheme (NDIS) so that you can plan and manage your life as you wish. We partner with you to support you in leading an independent and fulfilling life. Our supports can help you with:

Pruning and trimming, lawn mowing, weeding, raking, cutting large bushes & trees, yard tidy-up, moving pot plants, gutter cleaning, fix reticulation and removal of green waste. We can also provide advice and information on Water Wise garden design & landscaping.

Changing batteries in smoke alarms, replace door locks, check home security and cleaning windows. We can also provide financial support for adapting and modifying the home e.g. installing handrails.

We can visit you in the home to provide companionship to enhance social contact and minimise isolation. Our Social Centres offer a range of programs to meet new people, or catch up with old friends.

Our door-to-door transport services can assist you to go to the shops, medical appointments, and participate in social visits and events. We can also support you to find doctor’s rooms and navigate hospitals.

Chorus Social Centres

Ray Street Social Centre is located in Rockingham and offers a range of services to younger people with disabilities and their carers.

Services are planned to promote independence and community presence, they are person-centred and focus on re-gaining or maintaining your independence, autonomy and wellbeing. The services are tailored around your needs and are aimed at supporting you to learn new skills, develop new interests and achieve your goals.

What are the programs provided?
We offer services such as money management, cookery, food hygiene, arts and crafts, computer skills and various sporting activities.

We also offer a busy social calendar including discos, barbeques, quizzes and games. All of these activities are designed to offer you opportunities to interact, socialise and have fun with friends.

When are the programs provided?
Our services are flexible and are available during the day, evenings and weekends.

The Performance was an emotional experience, as it focused on the lives of people with disabilities, and we used drama combined with music to deliver our message.
